Enter NICU. Another nurse we have never had.
RT is now on NAVA mode of ventilation! The mode that uses an EDI catheter to read the signals his brain is sending to his diaphragm. NAVA level 2.4.
He is doing good on it, 22% oxygen! Nicely done!
I’m glad last night did not set him back! Was pretty anoxious coming in this morning.
Nurse says he had a decent night last night.
We look at the log book, looks like she was giving him stim and considering certain events brady desats when they actually were not.
Sometimes when he is relaxed his heart rate is around 120. She was considering 108 a brady. His monitor is set to not alarm unless his heart rate dips to below 80. He is older now and can have a lower heart rate normally.
Looks like on events she considered brady desats she gave him stimulation to try and get his heart rate back up… not necessary. Looking over the last couple days no one else has had to give stimulation. Oh well. That’s why we prefer nurses that know him.
He pooped last night at 4 also!
Blood gas before cares, looks really good!
Morning cares. Respiratory therapist weans to 21% oxygen. The respiratory therapist today has been there for almost all of RTs extubations, and intubations. He has definately made a bond with RT.
Lunch.
Back to NICU.
Hang out, RT is on 25%. He is having a really good day!
Cares, occupational therapist comes and does her thing. RT likes it mostly.
Then mom’s turn to hold him. She holds him swaddled. It goes smooth, he only desats once during the whole hold. He sleeps during most of it!
During the hold a PICU respiratory therapist comes by. He talks about what some differences are over there.
Oh by the way if I haven’t mentioned it one of those ongoing discussions we have been having with everyone. It is when is the right time to move to the PICU, because he is almost 41 weeks corrected gestation. More of a normal time to transition to the PICU these days. About 5 years ago, people stayed in the NICU and didn’t transfer.
Potentially moving to PICU Wednesday.
After the hold, cares. Nurse that only had him today has absolutely fallen in love with him! Many of the respiratory therapists work in PICU occasionally, and plan to visit anyways. Some of his nurses plan to visit too.
