30% oxygen! He is on his belly.
RT had a decent night except for two bigger events and a few smaller ones.
His vent settings weaned. His PC over PEEP has been turned down by 2 from 18 to 16!
This means the peak pressure (PIP) will be 24 instead of 26 on the big breaths the machine gives him.
He seems to be tolerating it well!
Have to explain to the nurse why RT’s schedule is the way it is again.
He gets cares at 0130, 0430, 0800, 1130, 1330, 1630, 2000, 2330.
She seems like she feels slightly inconvenienced by it. It is in RT’s orders so it has to be done.
It’s helping RT get better.
She asks if we want to do skin to skin early.
Nope I’m going to stick to our routine.
It works the way we do it.
Morning cares goes smooth. CPT, suctioned. The Occupational therapist comes. The nurse acts inconvenienced because it will take longer for cares. Not sure if she is joking or serious. Don’t care I encourage the occupational therapist to take her time, what she does is important and helps him.
Respiratory therapist wants to put him on his belly again.
I insist we are making big position changes, he has been on his belly, he should be on his back now because he will be on his belly again for skin to skin. Or else he will be on his tummy for 8 hours… put on his back. Weaned to 29% oxygen.
Lunch.
Back to NICU. Cares, skin to skin. My turn. Smooth transition.
Weaned to 27% oxygen.
Half way through bumped to 28% for a desat. He gets a dose of lasics to help with any additional water he can fit rid of because he is putting on weight so fast.
1630 skin up skin time is up.
Put him back, cares. Diaper is big and heavy just pee, I guess 135 grams. .. it’s a whopping 172 grams! Suppository.
Turned down to 27% oxygen!
Dinner.
Back to NUCU.
Shift change. Same nurse as last few nights!
Still on 27%!
2000 cares. Just pee, suppository didn’t work 😦
Respiratory therapist is the same one as last night. She seems more careful not to suction too deep tonight, so that’s good. Bumped up high on oxygen. BUT she weans him back down during cares thankfully. By the end 36% stI’ll higher than he has been all day but at least she didn’t just leave him as high as last night.
RT liked his CPT all day, but now he is angry and crying. I wonder if she is pushing too hard. The other respiratory therapists hold the weight of it and just rest it gently on him. I remember him getting angry with the other cpt device when she used it on him too.
I tell her to give him a brake because he is obviously pissed off and crying.
He immediately calms down.
Atleast she isn’t suctioning as overly deep.
After cares the nurse weans him down to 27% again!
Good boy.
Mom gives him some binky time, and book time!
He contines to fart and squirm, sounds like he is working on a poop.
We check. Yay! He pooped.
Another diaper change, decent poop.
Get him swaddled and settled back down.
Good boy!
Glad to see you under 30% or under all day with the exception of being bumped up unnecessarily.
Time to get some rest!
