Xray after intubation shows already slight improvement.
His oxygen needs slowly creep up with each care and don’t get turned down he sits at 96 spo2.

Cares are pretty standard, two small smears of poop, mostly pee, But not much .but with less fighting because of the fentanyl he is on making him almost asleep through cares.

Family rounds was long. We asked many questions about different things, prompting some change to the plan.

ND feeding tube being placed. Puts food past the stomach directly into the first portion of the small intestine. It should help with regurgitation or reflux.

Theofalin discontinued, Dr. doesn’t think he should be having apnea of prematurity any more. One side effect of thofalin could be relaxing of the upper sphincter of the stomach contributing to reflux.

Ask at what point we need to try something new if his oxygen keeps climbing.  Because a long time ago before the oscillator we hit 80% with the doctor not being told.

He wants to know if he is needing 50%

Ask about steroids, might switch to higher dose or stronger if we need to,  keep it in our pocket for now.

1700 cares. A bolus lorazepam to help keep him relaxed.

40% oxygen.

Xray to check placement of ND tube. I look at the xray, My guess is it is a little deep pull back 3 or 4cm.

Doctor comes to checks out the xray, says it’s slightly deep pull it back 4cm.

Talk to Dr.

I noticed some bumps on RT’s head,  he checks it out.  Could be a small amount of cradle cap, or just dry skin. Hasn’t had a bath in a bit. Not much to do. Mostly just takes time,  people try and treat it but really it usually doesn’t make it any better.

Nuse tonight another primary!

2000 care, feeds start,  continually feed now.  Starting off on half of what he was on.

Cares are now going to be stretched out to every 6 hours to let him rest more.

He is out of it because of pain meds.  We are glad he is comfortable but it’s hard to see him not be fiesty and his fighting self. But we don’t want him extubating himself.

Now up to 73% oxygen. he is getting turned up and not weaned. In the past he has shown he is able to regulate his spo2 and not high sat. Concerned he is getting worse.  I Point out he is on 73%. They begin turning him down .

Eventally back down to 32%.

Glad he has a primary tonight.

Going to try and get some rest,  mom and I slept poorly last night both of us having a bad feeling. Going to bed late, mom pumping and just not able to fall asleep.

You’ve made it through worse, I have faith you will make it through this too.

Love you RT.
