During surgery RT didn’t require too much additional oxygen. After surgery his oxygen needs went up during transfer to transport isolate and transfer to his bed in the NICU. By the time he was back and at the NICU after surgery he was requiring 100% oxygen.
Scary because he was close to room air just a few hours ago. Hopefully it’s just because he is disturbed, uncomfortable and whatnot.
After about an hour he settled down to 60%.
He suckles on his tube occasionally but other than that very little movement. He is out.
His has not opened his eyes, his eye lids are puffy. The dr warned us he has seen babies not open their eyes for a week after this surgery.
He is on fentanol to keep him comfortable. He is gettin eye drops that relax the muscles in his eyes to keep them from convulsing which can happen after this surgery. It also causes his pupils to dialate. He will have some sensitivity to light for a while, potentially bruising to his eyeballs/blood shot etc.
One thing he does do is hold our fingers and not let go. He wants to feel someone else is there. He is good at sleeping with a righ grip.
We hang out and do 2000 cares temp is stable, have been checking vitals after surgery every 15 min after surgery then every 30, then hourly. Blood pressure, blood gas, blood sugar.
Just pee. Eye drops put in.
After cares he settles down to 40% oxygen. Blood gas shows a low co2. Wean his respiration rate to 40 from 50 an tidal volume down to not low off as much co2.
We work up the courage to do a quick dinner then back to the NICU.
27% oxygen, weans settings again too! This is more reassuring that he isn’t regressing too far.
He is still mostly unconscious. We hang out quietly with RT and let him hold our fingers.
2200 cares. temp is slightly cold. Pee
2300 recheck temp. Slightly warmer. Oxygen turned up to 30%.
Way better than 100%. But still high on pressure and support and not initiating his own breaths. Probably because of all the sedation. hopefully he keeps requiring less and tomorrow can keep weaning settings and start progression on ventilator again, NAVA, NIV NAVA, CPAP.

We won’t see regression of ROP potentially for two to three weeks. But hopefully an immediate halt in worsening. With little to no change at next weeks appointment. So grateful to have gotten surgery the same day he was diagnosed.
