After another 8 hours bedside.
I have to follow that post with he is ok. But definately a set back.

I was going to write about how RT had been progressing during the day. But instead I go a call at 1140 from the hospital.
The call made the rest of the day seem insignificant. Continue reading “2340 August 19th 2016”
His head increased in circumference 3cm in 7 days.
Just had our weekly rounds, with RT’s team.
Cranial ultrasound doesn’t show an increase in ventricular size so that usually means the pressure isn’t increasing more in his head. Continue reading “August 18 2016 weekly rounds”
He is 4 weeks old today! 1235 grams! 27 and 6/7 gestation age.
When we arrived we were surprised he was having another cranial ultrasound. Continue reading “4 weeks old! August 18 2016”
Today we came in and his oxygen percent was 32%.(Room air is 21%) Peep down to 6 from 7, and NAVA level down to 2.2. From 2.5.
Positive End Expiratory Pressure (PEEP)
Essentially a constant pressure applied to the lungs to keep them from collapsing.
The NAVA level is the factor by which the Edi signal is multiplied to adjust the amount of assist delivered to the patient. This assist is thus proportional to the patient’s Edi and as such, it follows a physiological pattern
Essentially he doesn’t have to think as hard to breath because it is more sensitive to the signals being sent to his diaphragm. If he is sending good signal we can lower the amount we multiply it by.
Feeds are up to 12cc! He is tolerating them well. Continue reading “August 17 2016”
His IV for blood transfusion failed about 15 minutes into it last night. So they had to give him a new IV. The wonderful nurse was able to get it back into the same vane.
He was requiring higher oxygen percent last night and had been trending towards more de-sat so they did an X-ray. He wasn’t liking the peep being lower. Fluid was moving in around is lungs.
His weight on the board now reads 1160.
He has had to have his vent settings turned up a bit, they will likely do another blood transfusion again tonight because his red blood cell count is low again.
He is still extremely puffy from the edema. It seems to have migrated slightly, less around his eyes, more in his cheeks.
They upped his feeds to 9 cc now! Continue reading “August 15 2016”
I ask the nurse If we can give him more tastes again. I feel like it would be good to have him associate suckling and the taste of milk even if he can’t swallow it. She says it’s a good idea, at his morning feed I got to give RT tastes for my first time using a milk sponge on a stick. Continue reading “August 14th 2016”
On the board his weight is 1120 grams now.
They did another cranial unsound, to monitor his PVH-IVH it shows “dilatation” of the ventricles. I had to ask the Dr. What dilatation meant. My understanding is this Means the clotting in his brain is trying to escape and they have to expand to do so. So hopefully it doesn’t clog, because then we introduce other issues and will potentially need a stint in his head to relieve pressure. This would be indicated by rapidly enlarging ventricles and an increase in head size. They continue to monitor his head circumference closely. Continue reading “August 13 2016”
