August 24 2016 PM to 25 am

His oxygen needs have been fluctuating between 82 and 90% it seems very dependent on his positioning. Lots of tan fluids coming out during suctioning. Better out than in.

Infectious disease specialist came by and talked about the antibiotics he is on, and what they target.

Yeast is present in every persons lungs. But on a preemie it is a great concern due to the inability to fight infection adequately.

The yeast does not appear to actually be growing but is present at a elevated amount.

His white blood cell count is elevated.

His hematocrit is down to 24. Transfusion ordered.

New IV needs to go in. Hopefully smoother this time.

Dinner, pump, shower.

Another nurse that is familiar with him!

Back to NICU.

IV installed smoothly for blood transfusion. First try!

His needs have remained around the 80 percent range.  He looks agitated so we try repositioning him.  He seems less agitated after a good suctioning in his new position.

He calms down and we are able to slowly wean him down.

78,76,75, 74 73, 71. Getting nervous over how fast we are able to slowly to wean him.  Yes, still slow but faster than most times.  He continues to high sat on his sp02. His heart rate seems to be fluctuating between 174 to 187 then down to 160. Wonder if he is trying to poop.

I pull out my phone to work on my blog.

Earlier today I mentioned to mom I think he knows when phones are out.  Sometimes he just wants us to be present,  not texting about him,  writing about him,  or updating family.  He just wants us to sit.I feel like he can sense his surroundings somehow. I know it seems silly but it seems true.

He ofcourse goes down to 87 sp02 to make the alarm go off. The good range where he doesn’t alarm is 88-98. I tell him im turning off my phone. Off it goes. Then he comes up on his own which he likes to slowly do.

He proceeds to climb his sp02 back up to 99 and bounce between 99,100, amd 98.

If we can continue this trend it is what we need! Maybe we will be able to keep weaning him down then the night.

Maybe we have broken through… just maybe we can catch a break.

He seems stable. We give him a little extra time before weaning his oxygen anymore. He is doing amazing. We are nearing 20% less oxygen than at any time through the day.

Still not near room air.  But progressing,  micro preemie steps forward.

Progress

I ask RT if it’s OK if I go grab some water and have a rest room break.  He dings his 100 sp02 alarm a few times. Guess it will be ok.

Water, ice, pee.

Come back in the room,  RT is at 60% sp02. Nurse is by his bed.  Mom is watching.

I guess he didn’t like something.

It’s time to get a blood gas so the nurse put a  warmer on his heel.  It helps the blood come out easier.

Turns out he knows this means he’s going to get poked.
Nurse calls over other nurse and asks for respiratory therapist to be paged.  It’s the same few people from Fridays event.   I feel bad for them.

50,40. Bag comes out. Rescue breathing.

32.

40. 37

Nurse asks our nurse should I pull code tab? 50. Yes better safe than sorry.

Alarm goes off. I grab our bag and push our chairs away from incubator incase things get crazy.

He’s coming up. ., slowly.

Room fills with people within seconds, nurses doctor,  nurse practitioner,  charge nurse  respiraroty therapisr.

The lid of his incubator gets lifted for easier access. Check his vitals, he’s sounding ok, but wet.

Xray paged for emergency xray

60, 70 77, 80, 85,

Another team arrives with additional items to provide support.  Probably close to 20 people now.

89 95 100

Dr says we’re good they do some things and people file out.

We thank them all.

They ask if we are ok.  We are,  we ask if they are.

Mom and I remain calm.  Surprisingly calm.  Everyone responded excellently, so much support arrived so fast. It’s an impressive machine.

For some reason we knew he would be ok this time.

His food was pulled and thrown away.

Xray arrives. Images are taken.

Looks life we have lost most of the progress on lung recruitment again.

The Dr had the oscillator standing by this Dr has faith in the oscillator as a good tool, after showing us the xray. We agree. It is worth trying again.

Back onto the retro machine we go.

Large amounts come out during suctioning last few passes.

It seems he may have pushed up some of his food and aspirated some of the fluid.

No wonder he couldn’t breathe so quickly. Maybe this is partially what happened Friday.

Oscilator connected, set up.  Baby shaking good.

Weaned down quickly to 72%. Almost back to where we were…  Maybe to fast, desat,  back up a little.  Then slowly oxygen tornec back down. Blood gas looks good maybe even better.

We sit watch and wait.  Busy long day.  Glad we decided to come back and stay.

I didn’t have to get a dreaded phone call, or explained what happened,  or come in to see him yet again changed to the oscillator.

It was also nice to see how fast and proficiently everyone responded.

They all love him so much.

We all love him so much.

You all love him so much.

So many people have saved our tiny boys life.

One hour one minute one second.  Micro preemie steps, hopefully forward

Xray, and blood gas,  ordered for 0500

0130 blood transfusion started. No food for a bit.

Maybe we now are using the correct tool for the job for a bit.

Hopefully the next xray shows what we all pray to see. Progress

0200 Slowly weaning down 69%

Fairly stable.  Just his regular dips down to 84 sp02.

0340 he is in Amazing Hands, Time to try and rest.

Everyone loves you little boy

Prayers for progress.

Maybe…  I will have time to put up a few photos today.

Note to self. Planned phone call from Dr. In the morning,  For an update. Do not panic.
